This error happens because you need to patch qemu-kvm-rhev. The
patches aren't in our qemu-kvm-rhev yet (see rhvirt-patches for
current status), but I have a patched version of qemu here:

upstream qemu while using this local variables:
export PATH=/home/shahar/git/qemu:$PATH
export LIBGUESTFS_HV=/home/shahar/git/qemu/x86_64-softmmu/qemu-system-x86_64

Yes, known bug, of sorts. It's documented in the manual that it will
ask for the password twice. We can't really fix it because libvirt
doesn't talk to everyone else.
