It's hard to say exactly why - I'd need to see the full build log and the full log from 'libguestfs-test-tool'. You might try exploring the appliance using: ./run ./rescue/virt-rescue --scratch > <rescue> ls -l /usr/lib/ etc. Appliance building is much more robust with supermin 5 & more recent versions of libguestfs.
