This error happens because you need to patch qemu-kvm-rhev. The patches aren't in our qemu-kvm-rhev yet (see rhvirt-patches for current status), but I have a patched version of qemu here: https://www.redhat.com/archives/libguestfs/2014-May/msg00090.html
If reporting bugs, run virt-v2v with debugging enabled and include the complete output:

Yes, known bug, of sorts. It's documented in the manual that it will ask for the password twice. We can't really fix it because libvirt doesn't talk to everyone else.
